As a parent or legal guardian of a participant in the MEND Canada Basketball Clinic, I agree to the following code of conduct:

1. I will ensure that my child arrives at the clinic on time and is picked up promptly after the sessions.

2. I understand that the clinic is a safe and inclusive space, and I will encourage respectful behavior towards coaches, staff, and fellow participants.

3. I will support the coaches and staff in their efforts to provide a positive and constructive learning environment for all participants.

4. I understand that any form of bullying, harassment, or discriminatory behavior will not be tolerated.

5. I will inform the organizers of any medical conditions or special requirements that my child may have.

6. I understand that my child's participation is voluntary, and I will not hold MEND Canada or its affiliates responsible for any injuries that may occur during the clinic.

7. I grant permission for MEND Canada to use photographs or videos of my child for promotional purposes related to the basketball clinic.
